A monthly conference devoted to missed diagnoses has become a valuable means of assuring quality and providing continuing education at The Johns Hopkins Hospital Department of Radiology. Mistakes are collected and analyzed and methods of preventing errors are discussed. The subject matter is sensitive and requires special care. The two-year-old conference has proved useful in meeting the demands of the Joint Accreditation Commission for quality assurance.
