Abstract
Guided Care is a practical, interdisciplinary model of healthcare designed to improve the quality of life and efficiency of resource use for persons with medically complex health conditions. A Guided Care nurse works in partnership with several primary care physicians to provide coordinated, patient-centered, cost-effective care for fifty to sixty of their chronically ill patients. Guided Care and similar care coordination programs represent important opportunities to improve coordination of services for frail elders.
