Abstract
To the Editor: The report by Gottlieb and his colleagues (May 8 issue)* is an important contribution to the study of silent ischemia. However, there are several important methodologic and statistical flaws that tend to invalidate the authors' conclusion that “silent myocardial ischemia appears to identify a subset of patients who are at increased risk for early unfavorable outcomes.” In evaluating the clinical importance of silent ischemia, the authors use coronary-artery bypass surgery and percutaneous transluminal coronary angioplasty as unfavorable outcomes. They indicate that “revascularization” was performed in patients who had at least two episodes of angina at rest with.
