Abstract
A 74-year-old active man has long-standing bilateral pseudophakic glaucoma with visual field loss and has had filtration surgery in both eyes, with the right eye under tenuous control on a topical beta-blocker. The left eye has stable but advanced glaucoma damage with paracentral visual field loss. A previously placed PC IOL has decentered temporally. Intraocular pressure remains at 15 mm Hg, the filtration bleb is functioning moderately, and visual field loss appears stable. The patient has glare and reduced visual quality associated with the malpositioned IOL, particularly at night. Topical pilocarpine provides no relief. The patient requests assistance so he can drive at night. What would you suggest as the best course of action?
