Abstract

The frequency of bacterial complications in patients with renal insufficiency has stimulated the publication of multiple studies relating to the questions of antibiotic selection, dosing, metabolism, and pharmacokinetics in renal insufficiency. In this brief review our purpose is to define the general principles inherent in the development of guidelines for safe and effective therapy in renal insufficiency. We place particular emphasis on the alteration of systemic drug pharmacokinetics induced by renal failure and examine the alteration in tissue drug pharmacokinetics induced in the diseased target organ, the kidney.
