The differential diagnosis of the pediatric white matter lesions on brain MRI is often challenging. The purpose of this article is firstly to provide the systematic approach on brain MRI for diagnosis of the congenital metabolic disorders in children, along the process of myelination. In addition, we show the helpful MRI appearance to evaluate the severity of clinical prognosis in neonatal hypoxic ischemic encephalopathy. Finally, we provide the diagnostic clues on MRI for differentiation ADEM from pediatric multiple sclerosis.
